Completed Passport Application form,
Completed Online.
This form must be completed
online. The NY Department of State no longer accepts hand written
applications. (for those who have lost a valid passport, you must
also complete
Lost Passport Form DSP-64.
Mailing Address Must Be Left
Blank On Passport Forms. |

Government Fees are $195.00 for adults and $165.00
for children under the age of 16.
These fees are divided as follows:
-
Adult - $170.00 for "U.S. Department of State"
and $25.00 for the Post Office or the Clerk of
Court.

Minor - $140.00 for "U.S. Department of State"
and $25.00 for the Post Office or the Clerk of
Court.

Letters Of Authorization:
*Call the office for information on which Letter of
Authorization to use. |

Proof of departure date- computer generated flight itinerary from the
airline or a travel agency, copy of airline ticket or a corporate letter
stating US departure by private aircraft. |

Service Order Form. |

Fees and Payment (Service fees depend on the type of service required). |

Take the items 1-6 to Clerk of Court or Authorized Post Office and ask
them to endorse the documents and seal them in an envelope marked "TO BE
OPENED BY PASSPORT OFFICE PERSONNEL ONLY". This sealed envelope must be
presented to the passport office within 5 working days from execution.

Do not leave documents with clerk. |

Do not write checks until directed to do so by clerk. ALL GOVERNMENT
FEES MUST BE INSIDE THE SEALED ENVELOPE. |

You must present proof of identity such as a driver's license (which has
been issued for at least 6 months).

13 |
MINORS-All minors must appear in person at the post
office or the courthouse. Minors (under the age of 14) must be
accompanied by BOTH PARENTS or legal guardian with acceptable proof of
ID. If only one parent appears the other parent must sign a Statement of
Consent Form
DS-3053.

Send all the paperwork plus sealed envelope
to PASSPORT RUSH for processing. You must call to reserve a space for
processing if you need a same day service.
