You need a New Passport

If...

• you have never had a passport

• you have lost a valid passport

• your passport was issued more than 15 years ago

• your passport was issued prior to your 16th birthday

• your passport is damaged or mutilated

You need a Passport Renewal

If...

• you are able to submit your most recent US passport

• your most recent passport was issued in your current name

• your most recent passport was issued within the past 15 years

• your most recent passport was issued when you were 16 years of age or older

NOTE:  If you LOST your passport, you must apply for a NEW passport

You need a Second Valid Passport

If...

• you have a valid passport with travel restrictions due to existing visas within the passport

You need a Name Change

If...

• you have experienced a qualifying name changing event (Marriage, Divorce, etc...)
